Patients of former Ottumwa clinics must fill out paperwork

OTTUMWA — Patients of the former Ottumwa Family Practice and Ottumwa Internal Medicine North — which became Mercy Ottumwa Medical Clinic in July 2013 — may need to complete necessary paperwork before their next visit with their health care provider.

In order to comply with federal health information privacy regulations, patients must complete and sign a transfer of records form in order for the medical staff to have access to individual patient records.

Patients of Dr. Peter Reiter, Dr. Stan Blew, Dr. Robert Blommer, Dr. Florita Henderson, Dr. Brian Quinn, Molly Franke, PA-C, Ann Ruffalo, PA-C, Deb Stephenson, ARNP, Kim Brown, ARNP, and Stacy Greiner, ARNP, who have not been seen at the clinic since July 1, 2013, are encouraged to call the clinic at 641-682-4594 to make sure all the necessary paperwork is in order before any future appointments are scheduled.

Representatives from the clinic are available from 8 a.m.-5 p.m., Mondays-Fridays to take calls from patients regarding the transfer of their records.

“It is important for patients to complete the transfer of record form prior to visiting their health care provider,” said Lisa Bitner, Mercy Ottumwa Medical Clinic manager. “Transferring records now will not only help with future visits, but it is also important for patients who need prescription refills on a regular basis.”
